Adventist HealthCare seeks to hire an experienced Manager of Care Navigation for our Case Management department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.

As Manager of Care Navigation, you will:
• Perform duties to conduct and manage the day-to-day operations of the case management/social work functions, communicating with departmental staff to facilitate daily department functions
• Review and analyze activities, costs, operations, and forecast data to determine progress toward stated goals and statistical/financial purposes
• Promote compliance with federal and state regulations and contractual agreements
• Develop, implement, and maintain compliance with policies and procedures regarding case management
• Oversee compliance with the final rule and the clinical team
• Develop, implement, and maintain case management programs to facilitate the use of appropriate medical resources and decrease health plan financial exposure
• Facilitate ongoing communication between case management staff, members, contracted providers, and subsidiaries
• Develop staff skills and competencies through training and experience

Qualifications include:
• Bachelor’s degree in Nursing is required, Master’s degree preferred (required for Social Workers)
• Previous experience as a lead in a functional area, managing cross-functional teams on projects, or supervisory experience including hiring, training, assigning work, and managing the performance of staff is preferred
• 4-6 years of PPS/case management experience, and recent nursing experience in a rehabilitation setting
• Active Maryland RN license or Active Maryland LCSW-C license or LCSW with plans to achieve LCSW-C in 1 year
• Case Management Certification (CCM) preferred or matriculating for completion in the next year
• Active American Heart Association Basic Life Support (BLS) certification required

Pay Range:

$83,112.12 - $134,035.20

In cases where the range is displayed as a $0 amount or if the position is Per Diem (offering a fixed rate), salary discussions will occur during the screening process.
