Clinical Expediter (RN) - Capacity Command Center - Full-time

The Clinical Expediter is responsible for supporting hospital operations related to the coordination and facilitation of patient throughout and experience. This individual will focus on resolving delays in care and discharge barriers that are specific to improve patient experience and flow.  Additionally, the Clinical Expediter will work closely with Capacity Command Center (CCC) leadership to prioritize work for the day and is expected to utilize real-time and predictive information; he or she will monitor, triage, and address alerts on relevant dashboards.  The Clinical Expediter will investigate alerts using the electronic medical record and/or contacting the appropriate parties and determine the appropriate action for resolution.  Once a resolution path is decided, the Clinical Expediter will communicate /work in partnership with the local care team to support resolution. The Clinical Expeditor facilitates positive relationships that support both patient placement needs as well as any hospital operational needs related to resources to support patient care for placement.The Clinical Expediters are physically located within the Capacity Command Center (CCC) and may be assigned to resolve barriers within specific departments based on the needs of the organization.

• Minimum of five (5) years of relevant Nursing experience required
• Minimum of three (3) years of experience in acute care nursing.
• Epic EMR experience required.
• Ability to innovate, problem solve, build relationships, and negotiate is required.
• Leadership experience, patient placement knowledge and/or risk management strategies preferred
• Licensed RN in the State of Michigan required (Associates or Bachelors)
• Masters in Nursing/HCA preferred.  

• In depth understanding of the alerts, trends, flags and information provided on the Epic dashboards; able to identify patterns requiring proactive strategies to assure patient progression
• Triages alerts via investigation with the electronic medical record, conversations with the care areas, and other available resources
• Triages barriers to discharge to optimize impact on hospital capacity and free up high demand beds.
• In real time, expedites, prioritizes, synchronizes and mediates clinical issues, removes barriers to discharge and addresses delays in care
• Liaises with Capacity Command Center leadership, team members, and providers regarding clinical priorities and synchronization based on system needs.
• Collaborates with CCC operations and acute care leadership to identify process improvement strategies, reduce patient length of stay (LOS), trend delays, support quality initiatives and enhance patient safety
• Participates in department and organizational performance improvement activities
• Acts as a resource and liaison for providers, staff, patients and families
• Manages and documents all communication in a timely, efficient manner
• Facilitates Physician-to-Physician conversation when needed
• Strong listening skills; able to synthesize information to identify root cause
• Manages deviations from patient care pathways; resolves bottlenecks/variation in service level agreements, prioritizes with support services, and communicates with a variety of clinical departments.
• Collaborates with bed planners in assigning patients with complex clinical bed placement needs as appropriate
• Completes assignments and delegated responsibilities within established time frames
