Seeking a Registered Nurse to join our team in Camden and Burlington counties. The position is part-time (3 days per week), 6 weekends and one holiday requirement per year.

Job Responsibilities for a RN Hospice Float Nurse includes: 

  1. Performs initial and on-going patient assessments for comfort and palliative patient care
  2. Obtain patient’s history on admission
  3. Establishes and implements an individualized plan of care for each patient
  4. Assesses the need for patient and family education and provides education as needed
  5. Obtains appropriate physician orders to achieve symptom management
  6. Provides safe and appropriate skilled nursing care in accordance with protocols, policies, procedures and physician orders
  7. Manages and administers medications according to standards
  8. Documents in EMR in an accurate, concise and timely manner per standards
  9. Triages and arranges home visit schedule, demonstrating efficiency in time and mile management
  10. Meets or exceeds departmental productivity standards, including number of visits per week.
  11. Assigns patient care to other team members as appropriate (LPNs and CHHAs) and observes and evaluates the effectiveness of their work
  12. Makes referrals to additional IDT members and collaborates with social workers related to the involvement of additional IDT members
  13. Fosters IDT collaboration by initiating communication to address PFC needs/updates of plan of care
  14. Anticipates potential problems and establishes preventative measures
  15. Performs effectively in emergency situations
  16. Follows OSHA and agency regulations in the use of standard precautions, the management of controlled substances and biological
  17. Provide basic personal care and ADL’s when appropriate.
  18. Accept other assignments as deemed appropriate
