About the role

Job Summary:
The Home Health Aide (HHA) provides personal care services to patients in accordance with the plan of care established by the Registered Nurse and in compliance with Medicare Conditions of Participation. This position is designated for bathing visits only and requires strict adherence to safety, infection control, and documentation standards.

Essential Duties and Responsibilities:

  • Provide bathing assistance in accordance with the patient’s individualized plan of care.

  • Assist patients with shower, tub, or sponge baths as directed by the supervising nurse.

  • Perform additional personal care tasks related to bathing, including shampooing hair, skin care, oral hygiene, nail care (excluding clipping), and dressing after bathing.

  • Observe and report changes in the patient’s condition, skin integrity, or response to care to the supervising nurse promptly.

  • Maintain accurate, timely, and complete documentation of services provided in the medical record.

  • Adhere to infection prevention and control standards during all patient interactions.

  • Follow agency policies, procedures, and Medicare regulations.

  • Maintain patient dignity, comfort, and privacy at all times.

Qualifications:

  • Must have a 75 hour HHA Certification with Curriculum 
  • Successful completion of a state-approved Home Health Aide training program.

  • Minimum competency in personal care skills as demonstrated through initial and annual skills evaluations.

  • At least 12 hours of in-service training annually (as required by Medicare).

  • Ability to follow the patient’s plan of care and agency policies.

  • Strong communication and observation skills.

  • Previous experience providing personal care, preferably in home health or elder care.

  • Ability to assist clients with mobility and transfers as needed for bathing.

  • Compassionate, patient, and respectful demeanor

  • Reliable transportation and punctuality for scheduled bath visits.

Supervision:

  • Works under the direct supervision of a Registered Nurse (RN) or other licensed professional as required by Medicare CoPs.

  • Bath visits are assigned and scheduled by the supervising nurse or clinical manager.

Physical Requirements:

  • Must be able to safely lift, turn, and transfer patients as needed.

  • Must be able to stand, walk, and perform personal care duties for extended periods.

Work Schedule:

  • Flexible part-time scheduling based on client needs.

  • Bath visits typically range from 45 minutes to 1 hour per client.
