What we offer:

Job Functions: According to the mission and vision of the organization, supports the development and execution of strategies that impact positive health outcomes, lower health costs, and community engagement. This position is responsible for establishing and cultivating community events and health programming for families in need. Implement and maintain innovative health and wellness programming designed to encourage healthy lifestyles, increase family health education, and improve family health outcomes. Coordinates and oversee weekly Health and Wellness activities. Acts as a liaison with staff, community partners, and volunteers. Strategize, coordinate, and communicate with program leadership, volunteer services, food services, staff, and other internal departments who facilitate client health programming. This position is in-person, Pasco based, and 1-2 days a week in Tampa. Additional reporting days, weekend support, and duties beyond normal workday hours may be required as needed. Works closely with HR on Health and Wellness communications under the guidance of the Director of Health and Wellness.
Essential Responsibilities:
- Work with clients to develop and maintain Health and Wellness goals. Create objectives for their programs and track progress. Engage and support families as they try to increase their health and wellness outcomes.
- Assess the nutritional needs of clients and coordinate with the kitchen, outreach, and in-kind departments to ensure that nutritional needs are met. Education and support to clients on nutritional elements. Work with kitchen staff to ensure client dietary needs are accounted for.
- Represent Metropolitan Ministries at strategic and organization-supported community events, network meetings, and pursue partnerships that will enhance/support services and community involvement as determined by the organization's strategy. Provide tours of the campus facilities.
- Meet personal goals established in individual development plan, KPI’s, and based upon annual evaluations and through regular supervision. Actively contributes to achieving the team's and organizational targets and goals. Supports the team with other assigned tasks. Adheres to all policies of Metropolitan Ministries.
- Work with other departments to improve processes, meet growing needs, and involve supervisor as needed when there are barriers to reaching competing demands that prevent workable solutions.
- Maintain weekly tracking of client health progress and assist clients in preparation of medical procedures (pregnancy, surgery, etc.) and making accommodations within their living quarters for recovery.
- Maintain real-time notations related to Health and Wellness in the HMIS database.
- Facilitate Health and Wellness assessments of all families participating in residential programming. Provides food bins and YMCA memberships to clients when available.
- Reports to the Director of Health & Wellness for pervision and support.
Requirements
Education and Experience: Degree in Public Health, Health Education, or similar, along with a minimum of 2 years’ experience providing health and wellness services for at-risk populations. Must understand the inter-relationship of stress and trauma's impact on health outcomes.
