Family Medicine - NP or PA - Albany, MN

As a Family Practice Provider in our Albany Clinic, you will work as a part of a primary health care team in collaboration with a team of Physicians and Providers.  In this role, we are looking for an individual with a desire to work in a collaborative model.  You will be partnered with a Physician and will share and continue to grow a patient panel. 

Schedule:

• Full- time, up to 1.0 FTE, M-F 

Some responsibilities of the Family Practice Provider include:

• Chronic disease management

• Evaluating, implementing and managing patient treatment based on assessments of their needs

• Reviewing lab results and planning the course of action according to the results

• Providing preventative care to adults and children

• Educating patients on health and wellness

REQUIREMENTS

To be eligible for this position you must meet one of the following requirements;

• Certified as a Nurse Practitioner 

• Current Minnesota Physician Assistant Licensure 

We are hiring an applicant that meets or exceeds the following experience:

• Previous experience in Urgent Care
• Previous experience Family Medicine/Walk-In Clinic

Pay and Benefits

• Starting pay begins at $109,241.60 per year; exact wage determined by years of related experience

• Salary range:  $109,241.60 - $124,904 per year

• Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
