Description

Compensation: $28 per hour

Job Summary:
Meets with assigned participants regularly to do full assessment of participant’s physical, mental and social needs, income, eligibility for public benefits, life skills, competency, employability, educational needs, background, and other areas. Make referrals as needed. Assures that each participant utilizes program components that are consistent with the participant’s service plan. Understands and upholds SSVF Program guidelines.
Title: SSVF Lead Case Manager
Essential Duties and Responsibilities:
• Conduct initial and ongoing housing-focused assessment of participant needs
• Develop individualized housing stability plans based upon the participant’s strengths and weaknesses/barriers using the progressive assistance approach
• Review assigned case files weekly and conduct regular case management meetings
• Perform crisis intervention
• Works with Case Manager Supervisor to hire, train, manage, and evaluate program staff.
• Oversees program and contractual outcomes including responsibility for program planning and distribution of workload.
• Ensures program quality assurance to track program participant data for specific outcomes and reports.
• Provide support to all case managers. Supports collaboration and maintains a positive team dynamic.
• Explains the SSVF, VA and community services that are available and establish reasonable milestones for obtaining greater housing stability
• Monitor participants’ progress toward achieving the established goals, including requesting updates from participant service providers as necessary
• Coordinate supportive services offered within the agency and assist participants with accessing public benefits and mainstream community resources (e.g. medical, dental, mental health, legal, social services, veterans’ benefits, etc.)
• Maintain complete participant records in accordance with agency and best practice standards. Ensuring accuracy in documentation, reporting, and follow through.
• Maintains all required documentation in HMIS
• Attends weekly SSVF Staff Meeting and exchanges information with other staff regarding participant needs

• Attends all VVSD Staff trainings and meetings as required
• Develop and facilitate trainings and workshops for clients and SSVF staff
• Track and record outcome data for reporting purposes
• Conduct file audits
• Performs other duties as assigned by supervisor

Qualifications

Qualifications:
• Basic understanding of the military and the Veterans Administration
• Minimum of a bachelor’s in human services, Social Work or related field preferred
• Minimum of 2 years of paid case management experience in Human Services, Social Work or related field preferred
• Minimum of 1 year paid experience in a Housing First program
• Ability to prepare written reports and maintain participant documentation
• Be able to relate to individuals making significant life decisions, and be supportive of individuals' increasing independence
• Must be able to adhere to high standards of confidentiality
• Prefer prior experience working with homeless population
• Proficient skill level in Microsoft Office (Word, Excel, Power Point)
• Valid Drive License, in good standing
• Ability to prioritize competing responsibilities and thrive in challenging situations
• Able to change priorities at a moment’s notice
• Must be able to fluently read, write and communicate in English
• Must be able to comfortably, both mentally and physically, perform all aspects of job description
• Must be able to sit comfortably for long periods of time
• Knowledge of HMIS database a plus
• Knowledge of Housing First approach a plus
• Veteran Status a Plus!
• Must be able to pass background check and drug screen
Physical Requirements:
• Ability to bend and lift light objects
• Ability to sit for extended periods of time while viewing a computer monitor
• Dexterity of hands and fingers to operate a computer keyboard
• Capable of hearing and speaking to exchange information
• Seeing to read reports and data
• Ability to speak English
• Must be able to work in a noisy, busy environment.
