Overview

 

We are looking for a Patient Engagement Coordinator to join our AIM (Advanced Illness Management) Program team.  The Patient Engagement Coordinator reports directly to the Director of Patient Engagement. This role will coordinate initial and follow up outreach to patients for initial visit scheduling purposes.

  • Perform telephonic outreach to patients
  • Maintain communication with pending patients, their families, AIM sites, and referral sources and routinely follow the progress of prospective patients and those referrals not taken under care (NTUC).
  • Coordinate and maintain effective and efficient scheduling for the Nurse Practitioners to include geographical considerations that may impact time management and company cost.
  • Ensure and verify appropriate data entry for all key components related to a patient referral, to include but not limited to: patient legal name, payor, address, DOB, and telephone number.
  • Maintain patient status updates in all required systems, reports and EMRs
  • This position is 100% remote

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity.  Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

About You

  • High school degree or equivalent; College graduate preferred
  • Minimum of three years health care delivery or related business experience required
  • Strong knowledge of medical terminology and a customer service focus.
  • Experience in a role related to direct to customer interaction and engagement preferred
  • Effective data entry and word processing, problem-solving, human relations, and oral/written communications skills.
  • Experience with Computer applications and Electronic Medical Records.
  • Strong comfort level in performing telephonic outreach to patients whether referred from a warm hand off or a partner provided list. Ability to overcome patient perceived verbal rejections, and clearly communicate the tenants of the program and its benefits.
  • Ability to engage patients quickly through a warm, inviting phone voice.
  • Maintain standard of high quality customer service
