Licensed Vocational Nurse (LVN) - Hospice After Hours

Position Summary: We are seeking a compassionate and experienced After Hours Licensed Vocational Nurse (LVN).  By joining our team, you will be providing skilled care with an empathic approach to help patients and families cope with the End of Life process. 

• Deployed by the After Hours Triage Nurse when patient visits are required to be conducted during after hours
• Follows the care plan as instructed by the RN-Case Manager
• Communicates and works closely with the disciplinary team; Medical Director, Nurse Practitioner, RN, Social Worker, Chaplin and Hospice Aides
• Completes accurate and timely documentation within the company policies and procedures as well as applicable laws and regulations

Territory: Santa Clara County

Schedule: Monday - Sunday, 5:00pm to 8:00am, 7 days on/7 days off

Compensation: $350.00/shift stand by rate and $65.00/visit rate

Requirements

• Current CA LVN License
• Hospice LVN experience 
• Current AHA CPR certification
• Valid Driver's License
