Physical Therapist I - AVN, Per Diem Somerset County

If you answered yes to either question, then apply now!  Atlantic Health System is seeking a skilled and compassionate per diem Physical Therapist for the dynamic Atlantic Visiting Nurse Dept to work 8am-4pm in Somerset County. Ideal candidates will have at least one year of experience - preferable homecare - as this position will treat patients in their homes independently.

What your day might look like? 

• Conduct comprehensive evaluations and develop patient-centered treatment plans based upon evidenced based practice.
• Collaborate with physicians, case managers, and fellow rehab team members.
• Communicate with patients and families.

Is this you? 

• Graduate of an accredited college/university with a degree in Physical Therapy.
• Licensed to practice in New Jersey.
• Caring and compassionate therapist who is willing to advocate for your patients.
• Strong interest in participating in department improvements and participation hospital led initiatives.

 

Essential Functions/Responsibilities:

• Evaluate and treat patients in their homes

Minimum Education:

• High School Diploma or GED, required
• Bachelor's in Physical Therapy, required
• Master or Doctorate in Physical Therapy preferred

Minimum Experience:

• 1 year of experience in homecare, strongly preferred

License/Certification:

• NJ License in Physical Therapy
• BLS, preferred
