Position Overview:

  • Assesses and evaluates patients to determine appropriate treatment plan. Gathers and synthesizes data from patient and family, patient records, direct observation, and other healthcare providers.
  • Plans and provides occupational therapy treatment. Documents accurately all aspects of occupational therapy services in a timely and effective manner. Uses appropriate and thorough evaluation techniques and standardized testing as needed.
  • Works within the scope of services policy for occupational therapy.
  • Develops treatment plans and establishes measurable goals for patients. Implements treatment plan utilizing appropriate treatment procedures, exercise programs, and manual techniques.
  • Instructs patients, families, and other caregivers in therapies and activities to improve the patient's level of function, facilitate discharge, and maximize carryover of home instructions. Trains all parties in methods to maximize safety and functional abilities in performing activity of daily living (ADL) skills.
  • Coordinates care with other Capital Health interdisciplinary team members. Coordinates with healthcare providers and others outside of CH when needed including effective communication and advocacy for patients.
  • Ensures documents are timely including submission of attendance and charging records known as "billing sheets." Ensures documents are complete and effective in communicating essential aspects of care including evaluation, care, attendance, and discharge of the patient.
  • Ensures documentation of time spent with patient is accurate and clear, and statistical data such as charging, is accurate and documentation of a patient encounter includes date and time. Completes data reports, questionnaires, and surveys measuring patient outcomes and ensures it is complete as required.
  • Engages in professional development of self and staff. Participates in educational opportunities and self-directed learning. Participates in clinical education program for OT students.
  • Participates in committees, special projects, performance improvement (PI) activities, and presentations to community, including occasional participation in events outside of work hours as needed. Takes responsibility for problems or issues related to patient care or department operations.
  • Attempts to understand various points of view, employs positive problem solving techniques and works toward full resolution of issues.
  • Conducts assessment and coordination of home modification program including installation of equipment and other changes to support independence and safety.
  • Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment to participants.
  • Carries out accepted principles and practices of occupational therapy for prevention of functional decline, sensory and cognitive limitations, increase maintenance of strength and endurance, and provides treatment to increase self-care skills.
  • Performs other duties as assigned. Demonstrates willingness to adapt to changing departmental demands. Shifts among campuses to fulfill patient needs, as required.

MINIMUM REQUIREMENTS

  • Education: Graduation from accredited occupational therapy school or education approved by NJ State Occupational Therapy Advisory Council.
  • Experience:One year of experience with patient populations specific to the position required.
  • Other Credentials: AHA BLS - Healthcare Provider,Occupational Therapist License
  • Knowledge and Skills: Strong interpersonal skills and cultural competence. Excellent organizational skills.
  • Special Training: Ability to use other software as required to perform the essential functions of the job. Proficient with Microsoft Office Suite including Word, Excel, and Outlook. Ongoing continuing education courses.
  • Mental, Behavioral and Emotional Abilities: Possesses a high degree of personal accountability, responsibility, and independent decision making abilities with the skills to interpret programs, goals, objectives, policies and procedures of the organization in line with its mission, vision, and philosophy.
