Sangre de Cristo Community Care is seeking a dedicated and compassionate Hospice Admissions Nurse to join our dynamic team. In this role, you will be responsible for assessing and admitting patients to our hospice program. You will be the first contact for patients and families, making sure they understand the hospice philosophy and services we provide.

Responsibilities:

· Initiates the admission process by educating and informing the patient and family about the hospice benefit.

· Participates actively as a member of the Interdisciplinary Team to assist in developing and implementing the patient’s plan of care.

· Obtains consent, provides assessment, initiates the care plan, and helps optimize the patient and family’s comfort and quality of life.

Requirements

· Must be at least 21 years of age.

· A valid non-restricted RN license in the state of Colorado is required.

· Must have a valid non-restricted driver’s license.

· Flexibility and availability to work a weekend schedule if needed.

· At least 1 year of hospice or home health experience preferred.
