The Pediatric Complex Care Clinic provides a medical home for children with complex health conditions from birth to adolescence, helping families coordinate both acute and chronic care with WashU subspecialty providers, therapists and home care services.The Clinical Nurse Coordinator is responsible for coordinating the care of medically complex pediatric patients.
Job Description
Primary Duties & Responsibilities:
- Performs and documents assessment of the patient/family (biophysical, psychosocial, developmental, cultural, spiritual and environmental); collects and analyzes data from the medical record in order to coordinate patient care.
- Triages patient calls and MyChart communications. Provides appropriate counsel and education to patients/family regarding disease process and symptom management.
- Maintains medical records requiring appropriate and timely recording of patient information.
- Consults with members of the health care team, adjusting nursing care processes as indicated to ensure optimal patient outcomes.
- Assists the health care provider with examinations, procedures, prescription refills, and other processes related to direct patient care.
- Coordinates patient care, orders diagnostic tests, conveys findings to physician, coordinates patient appointments, and assists with referrals to outside sources; reviews, completes and submits documents as required by outside sources.
- Acts as liaison and team leader by facilitating communication and coordination of care between patients, families, and health team members.
- Other duties as outlined in specialty-specific job responsibilities.

Required Qualifications:
- Graduate of an accredited nursing program with two years of nursing experience.
- Current RN licensure in the state of Missouri and/or Illinois, depending on location, in accordance with the Compact Act.
- Basic Life Support Certification or obtains certification within one month of employment. Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements.
