Key Responsibilities
Key responsibilities and day to day responsibilities of this position:
- To
provide HIV non-medical case management to make sure Ryan White Part B clients
enter and remain in HIV medical care and have access to and are adherent on
their HIV medications.
- Provide case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program
- Perform client intake
- Assist in application and re-certification for the Part B Program
- Assess needs utilizing assessments as provided by the Program
- Develop care plan
- Coordinate care and benefits
- Provide and track referrals
- Provide internal and external resources
- Perform follow-up as needed and indicated by acuity and assessment scores.
Qualifications
Minimum or "day-one" qualifications:
- One
year of paid employment/professional experience providing case management
services, excluding internships, AND one of the following:
- Bachelor of Social Work.
Bachelor degree in health or
human services.
OR - Five years of paid
employment/professional experience providing case management services,
excluding internships.
- Working
knowledge of web-based applications and Google Workspace, Microsoft
Office, or similar.
- Ability to work independently with minimal
supervision
Preferred or trainable/on-the-job qualifications:
- Previous case management experience
- Working knowledge of HIV
- Knowledge of or experience with Ryan White Part B
- Familiarity with social service and support systems within Utah
- Experience with ClientTrack data management solution
