Key Responsibilities

Key responsibilities and day to day responsibilities of this position:

  • To provide HIV non-medical case management to make sure Ryan White Part B clients enter and remain in HIV medical care and have access to and are adherent on their HIV medications.     
  • Provide case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program
  • Perform client intake
  • Assist in application and re-certification for the Part B Program
  • Assess needs utilizing assessments as provided by the Program
  • Develop care plan
  • Coordinate care and benefits
  • Provide and track referrals
  • Provide internal and external resources
  • Perform follow-up as needed and indicated by acuity and assessment scores.

Qualifications

Minimum or "day-one" qualifications:

  • One year of paid employment/professional experience providing case management services, excluding internships, AND one of the following:
  • Bachelor of Social Work.
    Bachelor degree in health or human services.
    OR
  • Five years of paid employment/professional experience providing case management services, excluding internships.
  • Working knowledge of web-based applications and Google Workspace, Microsoft Office, or similar.
  • Ability to work independently with minimal supervision

Preferred or trainable/on-the-job qualifications: 

  • Previous case management experience
  • Working knowledge of HIV
  • Knowledge of or experience with Ryan White Part B
  • Familiarity with social service and support systems within Utah
  • Experience with ClientTrack data management solution
