The Behavioral Health Care Manager, Telephonic 2, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Behavioral Health Care Manager’s work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.The Behavioral Health Care Manager, Telephonic 2 employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.
Identifies and resolves barriers that hinder effective care.
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
May create member care plans.
Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
Follows established guidelines/procedures.
Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Required Qualifications
Must meet one (1) of the following:
Unrestricted active licensed Registered Nurse (RN) in the state of South Carolina with no active disciplinary action
Master's degree clinician in the field of social work, nursing, rehabilitation, psychology or related health and human services and licensed in South Carolina in one of the following, LCSW, LMSW, LMSW-ACP, CSW, LPC, LMFT without disciplinary action.
Must meet all of the following:
3+ years of clinical acute care experience in a behavioral health setting
Experience with serious mental illness specialty population
1 year Care Coordination/Case Management
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
Preferred Qualifications
For RN, BSN
Experience with case management, discharge planning and patient education for adult acute care
Managed care experience
Certified Case Manager (CCM)
Additional Information
Work Schedule: Monday - Friday; 8:00 AM - 5:00 PM Eastern Time
Location: Must reside in South Carolina
Travel: Ability to travel on site for trainings as needed

Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.

Interview Process
