Job Summary: Collects clinical data, aggregates and reports the data at the hospital and corporate levels as well as reports to federal and state agencies to gain accreditation, manage performance and capture reimbursement for services and to qualify for incentive programs. Works in collaboration with Information Technology and Quality Department to design reports and reporting tools. Monitors performance of the hospital and medical associates with regard to quality and assists with the resolution of problems and conflicts associated with data collection.
Promotes interdisciplinary collaboration with a team approach and champions excellence. Leads and facilitates performance improvement, safety, and systems change projects or programs, and teams for BHDM and medical providers to ensure compliance with process improvement requirements. Collaboratively designs processes to support quality and safety among eligible providers. Develops outcomes-driven processes and metrics for performance improvement. Develops and conducts education for the eligible providers and service line staff as indicated to support safety, quality and compliance. Delivers consistent, effective communication and performance feedback to leadership, staff, and other indicated parties. Communicates appropriate information to physician leaders. Assists with reviewing accreditation standards to ensure compliance in relation to the scope of assignment, advising of needed changes when identified. Collaborates with other Clinical Outcomes RN’s to assure consistency in quality across the continuum of care.
Minimum Education, Experience, Training, and Licensures Required:
Graduate from an Accredited School of Nursing. Associate’s degree minimum, Bachelor’s preferred.
Bachelor's Degree in Nursing or related field is preferred upon hire but must be enrolled in program within three years of hire with degree completed within five years of position hire date. Certification in associated area of expertise is encouraged. Maintains current license as a Registered Nurse through the Kentucky Board of Nursing. EXPERIENCE: Three years clinical experience required. A minimum of 2 years demonstrated experience in health informatics, clinical implementations, health analytics, healthcare quality or other related area is preferred. Knowledge and experience in the support of Process of Care and Outcome measures is preferred. Previous participation in performance improvement processes related to patient safety and/or clinical quality preferred. Strong project management and analytical skills desired.
