Youth Peer Advisor

• Provide Peer services to Sagamore Children’s Psychiatric Center (SCPC) patients and their families. Services will be rendered in the SCPC inpatient hospital at 197 Half Hollow Road, Dix Hills, NY 11746.
• Collaborates with SCPC program staff to help ensure the patient’s services and educational needs are met. This includes but is not limited to:
• Individualized intense outreach to help patients become engaged in service planning activities. Help patients identify and communicate needs. Educating patients about the service planning process and helping them develop the skills necessary to be active participants in their service plans and discharge plan. Empowering patients by helping them to develop an affective way to have a voice in their own recovery and teaching self-advocacy skills. Provide support and understanding from someone who was in their situation and understands firsthand their experiences. Develop, coordinate, and facilitate support groups as needed, including co-facilitating newly developed Youth Advisory Council and Super Skills Group. Connect to and encourage participation in groups, resources and services in their home community as available and appropriate. Treatment Team member. Share their own experience and positive outcome to offer hope that things can and will get better. Maintain patient confidentiality and adherence to HIPAA. Complete required documentation in a timely manner consistent with SCPC guidelines. Patients will be under the supervision / observation of SCPC staff while Peer services are provided.
