We have an exciting opportunity available for a HTN Community Health Worker to join the Community Engagement team at our Senate Location in Downtown Indianapolis. The role and activities of the Community Health Worker (CHW) are tailored to meet the unique needs of the community demographics serve throughout Marion County. The incumbent must have an in-depth knowledge of the communities they serve, identify health-related issues that affect a community, and take care of members of the community who may not be cared for by traditional medical facilities. The community health worker will offer culturally appropriate health education and information, help people get the care they need, give informal counseling and guidance on health behaviors, advocate for individual and community health needs, and provide direct services such as blood pressure screening, and other health related screenings. Some evenings, and weekend hours. 

Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up. Must have reliable transportation. Background in Medical terminology/medical (a plus). Proficiency in Spanish (a plus) 

Requirements

• High School Diploma or equivalent required.
• 0-3 years of experience required.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.
