About the role

The Day Support Program Coordinator is responsible for upholding Abound Health PA, LLC’s mission, vision and values by providing support, services, education and compassion to Service Recipients receiving Pennsylvania Waiver or Base Funded Services.

  • Conduct monitoring of activities in the community
  • Monitor and assist Direct Care Staff with running outcomes/goals
  • Assist Program Specialists/Case Supervisors with suggestions of changes to client’s goals
  • Perform any drills or site inspections as assigned
  • Inspect all operating equipment in Adult Training Facility (ATF) site(s) and ensure a safe and appropriate working environment, with a focus on safety, health and hygiene
  • Conduct weekly facility cleanliness inspection
  • Act as liaison with the cleaning company/personnel for the facility
  • Check and log water temps bi-monthly at Day Support site(s)
  • Check and log fridge and freezer temps daily at Day Support site(s)
  • Perform Quality and Satisfaction calls monthly to all families, guardians and/or consumers
  • Coordinate Event Calendar including community activity for Adult Training Facility program in collaboration with Day Program Director
  • Help coordinate Abound Health PA events
  • Inspect emergency/First-Aid kits of direct care staff quarterly
  • Assist Day Program Director with staffing patterns to ensure coverage and back up staff is available
  • Perform daily activities per program needs
  • Assist with annual inspection preparation as needed

GENERAL RESPONSIBILITIES

  • Provide privacy and confidentiality of all client records.
  • Implement the client’s individual goals/task analysis by using the interventions in the ISP and following appropriate documentation procedures
  • Communicate relevant consumer information to Supervisors, Support Coordinator, and/or Parents
  • Assure clients are free from abuse, mistreatment, and/or neglect and report any incidents of such to supervisor.
  • Treat each client with dignity and respect.
  • Develop and demonstrate a thorough understanding of Abound Health PA’s philosophy and purpose.
  • Provide a positive atmosphere which facilitates growth and learning of coping skills.
  • Perform duties as requested by Supervisor
