Clinical Pharmacist (Full-Time, Remote, North Carolina Based)

The Clinical Pharmacist is responsible for performing comprehensive medication reviews, participating in interdisciplinary team meetings, providing education, offering targeted case consultation, resolving medication access issues, and supporting pharmacy benefit management (PBM) operations. The Clinical Pharmacist will also contribute to population health and quality improvement projects to ensure health outcomes for Alliance Tailored Plan members, state-funded recipients, and Medicaid Direct beneficiaries.

This position will allow the successful candidate to work fully remote based anywhere within North Carolina; they will be required to report on-site to the Alliance office for business meetings and also travel to participate in community partnership meetings occasionally as needed.  

Responsibilities & Duties

Provide Comprehensive Medication Reviews and Consultations

• Review and screen high risk patients identified by internal reports and communicate recommendations to clinical staff 
• Perform medication review and reconciliation on referred cases
• Provide clinical pharmacy input, with behavioral health emphasis when needed to address complex cases and cases with complex co-morbidities
• Assist in the selection of the most cost effective, evidence-based medication regimen for patients and populations
• Participate in or lead interdisciplinary team meetings to develop a plan of care for high-risk members
• Work with care managers to improve patient access to medication, including but not limited to post discharge from EDs and inpatient units  

Provide Education and Training on Medication Management

• Serve as a drug information and therapeutic resource for Alliance employees, providers, and members
• Provide highly specialized education and training support for community coalitions, first-responders, and other local organizations in the broad area of advanced pharmacotherapy, with emphasis on behavioral health and substance abuse treatments
• Develop educational programs for Alliance clinical staff

Analyze Data on Medication Usage

• Utilize care management systems, pharmacy benefit manager (PBM) claims system software, and other clinical tools to uncover potential ineffective or high-risk prescribing practices and intervene as necessary
• Analyze and develop drug utilization review (DUR) reports to identify trends in medication usage and offer recommendations and focused interventions for improvement

Support Quality Improvement Efforts

• Assist in the development and implementation of medication related quality improvement plans
• Coordinate or participate in activities related to improving outcomes for targeted population health, treatment disparities, and disease management initiatives 
• Assist with Alliance initiatives to improve medication adherence within the covered population and to improve access and utilization of evidence-based drug therapies  

Support Pharmacy Benefits Management (PBM) Operations

• Participate in the Alliance Pharmacy and Therapeutics (P&T) Committee
• Help to resolve escalated member and provider grievances and concerns
• Resolve pharmacy claim denials for Tailored Plan members in a timely manner
• Provide decision support for member appeals, controlled substance lock-in changes, and other coverage requests
• Assist the pharmacy director in day-to-day tasks related to oversight of the Pharmacy Benefit Management contractor  

Minimum Requirements

Education & Experience

Doctor of Pharmacy Degree (PharmD) from an accredited College of Pharmacy and completion of a psychiatric clinical pharmacy residency. 

Or 

Completion of a PGY-1 clinical pharmacy residency and have a minimum of 4 years of experience as a clinical pharmacist that includes over 50% of time focused on direct patient care of behavioral health populations. 

Active, unencumbered licensure by North Carolina Board of Pharmacy is required.

Preferred:

Completion of a psychiatric clinical pharmacy residency

Board certification, including BCPP preferably or BCPS credentials

Knowledge, Skills, & Abilities

• Extensive knowledge of pharmacy and its related sub disciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug use review, quality assurance, legal, regulatory, and standards of practice.
