Job Summary

The Certified Wound, Ostomy & Continence Nurse (CWOCN/CWON) provides specialized nursing care for patients with complex wound, ostomy, and continence needs in the acute care setting. This role utilizes the nursing process to deliver patient- and family-centered care while addressing the physical, psychological, emotional, and social needs of a diverse patient population.

The CWOCN/CWON works independently and collaboratively with physicians, nurses, and the multidisciplinary healthcare team to assess, plan, implement, and evaluate care for patients requiring advanced wound, ostomy, and skin management. This position serves as a clinical expert, educator, consultant, and resource to promote evidence-based practices and optimal patient outcomes.

Essential Responsibilities

  • Provide expert assessment, management, and treatment recommendations for patients with complex wounds, ostomies, fistulas, pressure injuries, skin integrity issues, and urinary or fecal incontinence.

  • Collaborate with physicians, nursing staff, and interdisciplinary teams to develop and implement individualized care plans.

  • Perform comprehensive wound, ostomy, and continence assessments and participate in ongoing evaluation of patient outcomes.

  • Provide hands-on specialized nursing care and clinical guidance for patients with complex skin and wound care needs.

  • Educate patients, families, and caregivers on wound management, ostomy care, skin protection, and prevention strategies.

  • Serve as a clinical resource and consultant for nursing staff and healthcare providers regarding wound, ostomy, and continence care practices.

  • Assist with product selection and recommendations related to wound care supplies, ostomy products, and skin care management.

  • Mentor and educate staff on evidence-based wound prevention and treatment strategies.

  • Promote quality improvement initiatives related to wound care, pressure injury prevention, and patient safety.

  • Lead and coordinate wound care shared governance initiatives, including tracking and reporting wound-related data, prevention measures, and outcomes.

  • Participate in product evaluations, protocol development, and implementation of best practices related to wound and ostomy care.

  • Maintain current knowledge of clinical standards, evidence-based practices, and advancements within wound, ostomy, and continence nursing.

Schedule:

  • Monday through Friday
  • Four (4) 10-hour shifts per week
  • Scheduled day off will be determined collaboratively with the WOCN team and may vary from week to week based on department needs
  • Typical scheduled start time: 7:00 AM
  • Schedule flexibility may be required based on staffing needs, patient acuity, and patient volume.

Licenses & Certifications – Required

  • BLS (Basic Life Support) – American Heart Association (AHA)
  • CWON (Certified Wound Ostomy Nurse) – Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB) or
  • CWOCN (Certified Wound, Ostomy & Continence Nurse) – Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB)
  • RN (Registered Nurse) License – State of Pennsylvania (PA)
