Job Summary

• The delivery system practiced at FHN Memorial Hospital employs the concepts of differentiated nursing incorporating the PRIDE Standards as set forth by FHN.
• Provides direct nursing care to identified patient population.
• Delivers direct patient care to achieve optimal patient outcomes.
• Demonstrates competencies in accordance with specific performance standards.

Job Responsibilities

• Other duties may be assigned.
• Demonstrates clinical competency in providing age specific care to patients in designated areas of practice.
• Performs therapeutic measures and assists with ADL’s.
• Assures accurate and timely medication administration.
• Assists with completion of shift assessments.
• Evaluates, reports and documents patient outcomes.
• Assists with patient education and psychosocial needs.
• Assists with discharge instructions.
• Delegates appropriate tasks to unlicensed staff Responds to customer requests/concerns.
• Performs phlebotomy on all age groups.
• Displays competency for the Licensed Practical Nurse role as evidenced in the annual performance appraisal following the FHN PRIDE Standards.
• Monitors and evaluates the work of unlicensed staff.

Requirements

Education:  Graduate from an accredited LPN program
Required Experience:   Zero to six months related experience and/or training or equivalent combination of education and experience.

Licensure/Certification/Registration:  Current licensure as an RN/LPN in Illinois; CPR certification; Pharmacology (will be provided if not present at hire).

• Older Adult: > 60 years
