We're seeking a dedicated and holistically-minded Behavioral Health Professional for our LGBTQIA2S+ Clinical Services Specialist - Choice Care Coordinator role at Multnomah County Health Department, Behavioral Health Division. This is an extraordinary opportunity to deliver crucial care coordination and support to individuals with complex mental health and substance use needs, ensuring they receive culturally competent and comprehensive services and promoting community-based solutions.

Knowledge, Skills and Abilities: This position requires extensive knowledge of and experience working with the LGBTQIA2S+ community, as well as broad knowledge of culturally matching community resources. This role provides outreach, engagement, and care coordination to help LGBTQIA2S+ individuals overcome barriers to health care. The role also requires an in-depth understanding of how to support the needs of LGBTQIA2S+ people with a multitude of intersecting identities (ex: BIPOC, immigrant/refugee, differently-abled, etc). This is demonstrated by knowledge of cultural norms within the LGBTQIA2S+ community, as well as having a close affiliation with and lived experience in the LGBTQIA2S+ community.
 

Teleworking: At this time, this position is designated to “routine telework,” meaning you will be working remotely (from home and community) most of the time; a hybrid schedule also is available. As with all routine telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.

As the LGBTQIA2S+ Clinical Services Specialist - Choice Care Coordinator you will:

Care Coordination

• Provide intensive care coordination, case coordination, and evaluations for clients with complex mental health and healthcare needs.

• Ensure timely and effective coordination with all partner agencies and respond to requests for intensive care coordination services.

• Initiate and participate in staffings to problem-solve barriers to services and discharge planning.

• Arrange specialty mental health services and exchange risk assessments to support client treatment plans.

• Track client information, treatment needs, and partner decisions to meet funding and reporting requirements.

• Provide clinical recommendations to community partners while working in a culturally competent manner with the LGBTQIA2S+ and non-binary communities.

Documentation

• Document all coordination efforts in the Evolv NX electronic health record.

• Record communication with service/placement providers, member/provider contacts, and meeting attendance/staffing.

• Complete assessments using clinical information to determine level of need and appropriate placement.

• Identify gaps in care based on clinical assessments.

• Provide clinical input regarding client placement and service needs.

• Work in a culturally competent manner with the LGBTQIA2S+ and non-binary communities.

Community Relationships

• Nurture and develop positive, collaborative relationships with community partners.

• Maintain partnerships with contracted agencies, healthcare providers, and social service providers as a liaison for the Behavioral Health Division (BHD).

• Build mutually beneficial relationships with providers, especially those offering culturally specific services, and document community needs.

• Share updates and feedback on community relationships with leadership to align program goals with external and funding needs.
