Description

   

To assess and identify patient needs to ensure that safety and high quality care is provided through nursing interventions, education, patient advocacy and coordination of services. 

  

Essential Position Duties

  1. Provide smooth and timely flow of patients in accordance with the COHC patient flow guidelines responding to high volumes of patients in  short-term time spans. 
  2. Demonstrate, understand and perform triage activities for all walk-in patients and medical phone inquiries; using reasonable and astute clinical judgments in order to expedite the appropriate care and treatment of patients. 
  3. Provide health care advice through telephone calls or the Internet.
  4. Provide nursing care based on physician and mid-level provider orders, to meet the needs (physical, mental, and emotional) of patients.
  5. Coordinate support staff in clinical area in delivery of care to patients in collaboration with Providers. 
  6. Work with the Chief Nursing Officer to ensure the clinic is adequately staffed for quality care delivery and the promotion of continuity of care. 
  7. Maintain  universal precautions and infection control practices.
  8. Support  and ensure accuracy of medical assistant who obtain initial information from patients (i.e.: vital signs, symptoms, and complaints) through interviewing patients and reviewing documentation. 
  9. Supervise  work of the LPN. 
  10. Prepare patients for examination and assist physician upon request. 
  11. Be able to assess patient condition, recognize adverse signs and symptoms,  and react swiftly in emergency situations.
  12. Administer medications per facility protocols, as prescribed by provider.
  13. Perform a range of nursing procedures, including dressing changes, suture removal, oxygen administration, wound irrigations and catheterizations. 
  14. Provide health materials, resources and education to patients and their families for informational purposes according to need and learning capacity. Assist the providers as required in ensuring all patient information on electronic encounter documentation is complete and accurate.
  15. Participate in the ongoing Performance Improvement initiatives between Home Health  Care Agency Subcontractor(s) and the COHC. Ensure that all required  reports, clinical and administrative documentation is completed in an  accurate, timely and complete fashion. 
  16. Participate in the management of day to day COHC operations. 
  17. Develop  and provide individualized and group counseling, guidance, and health instruction with the focus on disease prevention and health maintenance.
  18. Teach patients and/or families proper home health care activities, such as giving injections, taking blood pressures, changing would dressings, etc.
  19. Possess  knowledge of pharmacological agents used in patient treatment, including desired effects, side effects, complications, and patient usage  considerations.
  20. Possess knowledge of the basic concepts of customer service techniques related to age-specific population.
  21. Comply with all Federal, state, local, Joint Commission, Occupational Safety and Health Administration (OSHA), DPH, Subcontractor safety and operational regulations, directives and standards.
  22. Maintain confidentiality of all information and support patients’ privacy, rights, and safety.
  23. Provide coverage for other clinical areas ( as required)
  24. Perform other work-related duties as assigned. 
  25. Must maintain appropriate attendance.

  • Flexible hours as required as well as rotation on Saturdays.

Compliance Responsibilities

  • In collaboration with the Chief Nursing Officer, assists with ensuring quality and regulatory compliance within the nursing department and organizationally as appropriate. 
  • Complies with applicable legal requirements, standards, policies and procedures including but not limited those within the Compliance Process, Code of Conduct, and HIPAA. 
  • Participates in required orientation and training programs, as required. 
  • Reports concerns and suspected incidences of non-compliance in accordance with COHC Compliance Reporting Process. 
  • Cooperates with monitoring and audit functions and investigations. 
  • Participates, as requested, in process improvement responsibilities.


  

POSITION QUALIFICATIONS

  • Demonstrated effective verbal and  written communication skills along with proper telephone etiquette.
  • Working knowledge of Microsoft Office Software and basic computer maneuverability.
  • Knowledge of pharmacology.
  • Ability to efficiently function in fast paced work environment and multitask.
