Overview

Private Duty LPN – Full-Time Day Shifts

Location: Kenton, TN

 

Overview:Adoration Home Health & Hospice is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to provide specialized one-on-one care for patients with G-Tube needs in their homes. This full-time role offers a unique rotating schedule, providing flexibility and meaningful patient interaction.

 

Schedule:

• Day Shifts: 6:00 AM – 6:00 PM
• Rotating ¾ Split:
  • Week 1: 36 hours
  • Week 2: 48 hours
• Every other weekend

Positions Available:

• Two LPNs needed for day shifts

Qualifications:

• Active LPN license in Tennessee
• Current CPR certification
• Experience with G-Tube care preferred
• Strong organizational and communication skills

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family teaching according to plan outlined by the RN
• Monitor patient's vital signs and make specific measurements and observations
• Monitor and record intake and output. Report significant changes immediately
• Observe, assess and record patient's physical and mental condition and reports any changes
• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

• Licensed Practical Nurse / LPN license in good standing
• Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
• Active CPR certification
• Familiarity with electronic health records
• Strong problem solving and critical thinking skills
• Reliable transportation, valid driver's license and current automobile insurance
