About the Role

As a Physical Therapist in Union Grove, you will help residents improve mobility, regain function, and maintain independence. You’ll develop individualized treatment plans, collaborate with a skilled interdisciplinary team, and deliver meaningful care that changes lives.

Key Responsibilities

• Conduct comprehensive assessments of residents’ physical function and mobility

• Develop and implement individualized treatment plans

• Provide skilled physical therapy interventions

• Educate patients and caregivers on mobility strategies and fall prevention

• Recommend appropriate assistive devices and modifications

• Maintain accurate, timely clinical documentation

• Collaborate with nursing and therapy colleagues to ensure holistic care

Qualifications

• Graduate of an accredited program with a bachelor’s, master’s, or doctoral degree in Physical Therapy

• Licensed or eligible for licensure in the State of Wisconsin

• Strong clinical, communication, and organizational skills

• A passion for working with older adults in a skilled nursing or rehabilitation setting
