Patient Care Coordinator

The Patient Care Coordinator is a vital member of the Population Health Department, collaborating closely with the Quality Improvement Supervisor and Specialists to support initiatives that enhance patient care and improve quality outcomes. This role involves collecting and reviewing patient health data, patient scheduling, analyzing health risks, coordinating referrals, and managing patient registration (if needed). The coordinator also facilitates patient communications, supports care plans, and helps patients to ensure a positive experience and adherence to treatment plans. Additionally, the Patient Care Coordinator participates in meetings with payors alongside the Quality Improvement Specialist and contributes to programs aimed at improving Pacific Medical Centers’ quality measures.
Essential Functions
- Reach out to patients to schedule appointments and referrals; respond to patient callbacks in a timely and professional manner.
- Provide exceptional customer service and demonstrate caring behavior in all interactions.
- Collaborate with the population health team, patients, caregivers, families, and healthcare providers to identify needs, answer questions, resolve issues, and provide information.
- Accurately register patients in the electronic medical record (EMR); verify demographic information, insurance eligibility, and primary care provider (PCP) details.
- Actively obtain and update required patient demographic and insurance data.
- Obtain pertinent patient insurance information in order to facilitate verification of eligibility and benefits. May access insurance verification portals such as One Health Port
- Schedule appointments and document all encounters and interactions into EPIC.
- Request and abstract medical records as needed.
- Create new patient intake records and verify all required information, including insurance eligibility and authorizations if needed.
- Distribute patient education materials to meet regulatory requirements (e.g., HIPAA, Patient Rights).
- Participate in interdisciplinary quality improvement, problem identification and resolution initiatives and Population Health projects with PacMed Health clinics and payers.
- Assist with EPIC downtime procedures and ensure accurate data entry during recovery processes.
- Provide clerical support such as typing correspondence, operating office equipment, managing mail and faxes, maintaining files, sorting and distributing Interoffice and US Mail and scanning documents into the Electronic Medical record (EMR) and payer portals.
- Organize and prioritize work to manage multiple tasks concurrently and respond appropriately to requests from team members, healthcare staff, and patients.
- Attend department meetings and ongoing education workshops.
- Maintain a clean, organized, and well-stocked work area.
- Provide timely responses to communications via Microsoft Teams and email.
- Provide backup support for team members as requested by the manager.
- Collect, record, and enter daily and monthly productivity statistics and timecard data.
- Adhere to federal HIPAA guidelines to protect patient privacy.
- Demonstrate understanding of departmental and organizational operations by following established policies and procedures.
- Perform other duties as assigned.
Required qualifications:
- 1 year Medical/healthcare setting; or 2 years Customer service role
- 6 months recent receptionist or front desk experience
Preferred qualifications:
- 3 months Related experience (recurring appointments)
- Experience with medical terminology and basic medical record processes
