Job Summary

We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive support and guidance to patients in need of medical and mental health services. Your role will involve coordinating care, advocating for patients, and ensuring that all necessary resources are accessed

Duties

  • Conduct thorough assessments of patients' needs and develop individualized care plans
  • Collaborate with healthcare professionals to coordinate and monitor the delivery of services
  • Provide counseling and support to patients, addressing their mental health and addiction concerns
  • Assist with discharge planning, ensuring a smooth transition from hospital to home or other care facilities
  • Maintain accurate and up-to-date medical records and documentation
  • Advocate for patients' rights and ensure they receive appropriate care within the managed care system
  • Utilize effective communication skills to establish rapport with patients, families, and healthcare providers
  • Stay informed about current trends and best practices in case management

Skills

  • Strong knowledge of hospital procedures and medical terminology
  • Experience in case management or related field
  • Proficiency in utilizing electronic medical record systems
  • Ability to provide mental health counseling and support to patients
  • Excellent organizational skills with the ability to prioritize tasks effectively
  • Knowledge of utilization management principles and practices
  • Familiarity with pediatric care is a plus
  • Understanding addiction counseling is beneficial
