About the role

Schedule: Full time, 80 hours per pay period – Monday to Friday – 8 a.m. to 4:30 p.m.

Pay range: $41/hour to $52/hour (hourly non-exempt) based on qualifications, experience, and location.

Position Territory: Northwest Territory

      As a Physical Therapist Liaison, you serve as a clinical expert, educator, and connector between discipline-specific field staff and leadership. This role contributes to operational steering committees, supports process improvement, and ensures compliance with regulatory and organizational standards. In addition, the liaison provides direct client care, documentation, and follow-up while collaborating across teams to ensure consistency and excellence in therapy services. 

      As a Physical Therapist Liaison, you will:  

      • Clinical Leadership & Best Practices – Serve as a resource and subject matter expert to therapy peers; provide feedback and input on treatment planning, regulations, and evidence-based practice to strengthen care delivery. 
      • Collaboration & Education – Partner with education leaders, informatics specialists, and fellow liaisons to shape orientation, ongoing training, and compliance tools that support therapist success across all service areas. 
      • Direct Client Care & Documentation – Evaluate clients, develop and implement treatment plans, provide high-quality therapy services, and ensure timely documentation in the EMR in compliance with payer and regulatory requirements.  
      • Process Improvement & Communication – Gather feedback from field staff, participate in operational steering committees, and generate innovative solutions to improve systems, software, and interdisciplinary communication.  
      • Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive. 

      This job also requires:  

      • Current licensure as a Physical Therapist or Physical Therapist Assistant in Minnesota and North Dakota.  
      • Minimum of 3 years clinical practice experience, with at least 1 year in a community-based setting.  
      • CPR certification within 6 months of hire.  
      • Valid driver's license and insured personal vehicle. 
      • Strong knowledge of home care regulations, fiscal responsibility, and operational excellence standards.  
      • Ability to pass state mandated background checks. 
      • Physical capability to perform all essential job functions.  
      • Ability to read, write, and speak English to ensure effective communication with team members, clients, and visitors.
