Job Summary

• The Interim Step-Down Housing and Supportive Services Program (ISHSSP) and County of Los Angeles Justice, Care and Opportunities Department (JCOD) Specialized Treatment for Optimized Programming (STOP) Case Manager (also known as the ISHSSP/JCOD-STOP Case Manager “and Placements”) is the direct provider of casework and placement services to current and prospective students/participants. Responsibilities can include but are not limited to intake, initial assessment, initial placement in community-based services, transportation, case planning, case management, and collaborating with Community Based Providers (CBP), the County or LA Juvenile Probation Dept. and County of LA Courts, about progress and adjustments as needed

Case Management & Participant Support:

• 
  
• ·        Provide interactive services to the ISHSSP and/or JCOD STOP students/participants.
• ·        Develop Case Management Plans (CMP) for participants within 30 days of placement, ensuring goals and objectives align with assessment results.
• ·        Monitor participant’s progress throughout all program phases and adjust plans as needed.
• ·        Provide a minimum of two (2) face-to-face case management sessions each month (one may be virtual based on work schedules, geographic location, etc.).
• ·        Make appropriate referrals to community-based providers and outside agencies as necessary.
• ·        Develop discharge and aftercare plans to support successful program transitions/completions.

Documentation & Compliance:

• ·        Maintain comprehensive and accurate case records, progress notes, all required elements of the CMP, and statistical information for ISHSSP/JCOD STOP participants in both participant files and a data management system.
• ·        Document the evidence-based assessment results within the CMP, enter it into a data management system and share with the participant and community-based providers.
• Ensure CBP subcontractors follow policies and procedures to meet all contract(s) requirements

Collaboration & Communication:

• ·        Maintain strong, collaborative relationships with ISHSSP/JCOD STOP Contract Compliance Manager and/or other Amity Faculty, Probation and Criminal Justice liaisons, subcontracted CBP programs, and JCOD ISHSSP/STOP participants.
• ·        Communicate effectively and professionally with all relevant parties to support participant success

Youth Engagement & Skill Building:

• ·        Engage participants via CBP provider staff using effective communication, active listening, and conflict meditation skills.
• ·        Work in conjunction with CBP staff to model positive behavior and provide encouragement during challenging situations.
• ·        Equip CBP’s/participants with tools and skills to support prosocial decision-making and long-term success.
• Guide CBP’s and motive at-risk youth toward healthier lifestyles and improved community integration

Education and Experience:

• Required:
• ·        High School Diploma or equivalent
• ·        Experience working in substance abuse program(s), with criminal justice , or with at-risk youth offenders, or
• ·        Experience in providing case management services.
• ·        Valid California driver’s license and eligibility for Amity vehicle insurance.
• Preferred:
• ·        Two (2) years of experience working with at-risk youth in the provision of treatment, housing, and/or case management services.
• ·        Two (2) years of experience working with criminal justice populations in the provision of case management services.
• ·        Current Substance Use Disorder (SUD) Certifi
