This job assumes responsibility for the organization, coordination and implementation of an activities program for the patients on the Skilled Nursing Facility. Coordinates this function with the other therapies on the Skilled Nursing Team. Responsible for accurate and timely record keeping and documentation of patient activities.

Education

Required - High School diploma or equivalent


Work Experience
Required - Experience in activities therapy.


Certifications

Required - Certification as a Patient Activities Coordinator from a state approved activity program training course

Job Duties

  • Provides a functional activity therapy program for all patients on the unit.
  • Directs and supervises the activities program.
  • Completes written assessments on patients.
  • Participates in interdisciplinary team meetings.
  • Provides unit personnel with better understanding of activities and is responsible for communication of patient’s participation in activity program.
  • Develops and maintains a list of community resources which will provide recreational and personal services.
  • Assumes responsibility for the maintenance of activities equipment and supplies
  • Other related duties as required.
