Work Schedule

100% FTE, day shift. Weekday AM shifts. You will work at the Primary Care Clinic locations located in greater the Madison, WI area and the Administrative Office Building located in Middleton, WI. You may have the opportunity for some remote work. Hours may vary based on the operational needs of the department.

We are seeking a Clinical Pharmacist who: 

  • Demonstrates expertise in clinical practice for primary care patients and manages medication services utilizing delegation protocols in hypertension, diabetes, hyperlipidemia, and weight loss.
  • Supports providers and clinic team by offering support for complex cases and leads medication safety and performance improvement initiatives.
  • Collaborates with the healthcare team to optimize drug therapy and supports clinical outcomes.
  • Leads training on ambulatory care pharmacology, acts as a preceptor for students and residents, and contributes to the pharmacy profession's advancement through health system committee involvement and scholarly activities.
  • Delivers remarkable patient care by prioritizing empathy, communication, and collaboration.

 

PGY2 in Ambulatory Care or Specialty Pharmacy Residency preferred or equivalent work experience. BCACP preferred but not required.

Education:

Minimum – Bachelor’s degree in Pharmacy or a PharmD degree

Preferred – Completion of an ASHP Accredited Residency training program

 

Work Experience:

Minimum – One year of clinical patient care and orders management experience

Preferred – Three years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program

 

Licenses and Certifications:

Minimum – Licensed to practice pharmacy in the State of Wisconsin or must obtain within 90 days of hire date. Please see license and certification table below for BLS, ACLS and PALS. These are required or must be obtained within 90 days for BLS and 180 days for ACLS/PALS

Preferred – Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.).

UW Health Pharmacy Services Building – a non-clinical location in Middleton that houses nearly 100 Pharmacy professionals. These employees perform various services and duties, including pharmacy administration, Pharmacy Technician Training Program, pharmacy primary care services, medication prior authorization, sterile and non-sterile compounding, specialty/mail service program and call center.
