We're seeking a dedicated and empathetic Nurse Care Manager to:

• Provide education and support to clients with complex medical condition
• Collaborate with internal and external treatment teams to develop personalized treatment plans
• Facilitate health education groups and wellness initiatives
• Participate in treatment plan development and implementation

Key Responsibilities:

• Develop and implement personalized treatment plans in collaboration with internal and external treatment teams
• Provide education and support to clients with complex medical conditions, facilitating their wellness and prevention initiatives
• Facilitate health education groups and wellness initiatives, promoting healthy lifestyle choices
• Participate in treatment plan development and implementation, ensuring comprehensive care
• Collaborate with treatment team members to identify and address client needs

Education, Experience, and/or Credential Qualifications:

• Active RN license
• Experience with the IDD population preferred

Additional Qualifications:

Excellent written and verbal communication skills

Ability to implement new innovative systems

Ability to make decisions based on critical thinking and nursing process

Successful completion of background check, including criminal record, driving record, abuse/neglect, and fingerprint check

Current driver's license, acceptable driving record, and current auto insurance

Physical Requirements:

• ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.
• Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
• Sedentary work involves sitting most of the time but may involve walking or standing for brief
