About the role

The Mary T Family of Companies is seeking a part-time Occupational Therapist!

The Occupational Therapist will provide skilled therapy intervention and treatment to patients in the home setting.  This care includes appropriate utilization of treatment plans and assessment skills, careful monitoring of the patient’s status and progress toward goals, as well as compliance with physicians/ primary care provider’s (PCP) orders. The position works cooperatively with RN case managers, physicians/PCP, other home health staff and supervises the physical therapy assistant staff.   Responsible for educating patients and caregivers while preparing the patient/ caregiver for discharge as appropriate.

SCHEDULE: Part-time hours, daytime, Monday - Friday - Schedule can be VERY FLEXIBLE to meet your needs/preferences!

LOCATION: We have an office in Coon Rapids, MN, serving clients in the surrounding area. We also have a secondary office in North Branch, MN, serving clients in the surrounding area. Candidates that are willing to work with patients in both offices would have opportunities for more hours.

ESSENTIAL FUNCTIONS AND JOB DUTIES:

  • Implement occupational therapy services in accordance with physician’s plan of treatment.
  • Assist the physician/ (PCP) in the evaluation and assessment of the patient’s level of functioning by applying appropriate tests and assisting in the development and revision of the plan of care.
  • Treat the patient through use of therapeutic activities designed to restore function and self-care activities to improve function.       
  • Observe, record and report the patient’s reaction to treatment and any changes in the patient’s condition to the physician/PCP, Clinical Manager and Case Manager.
  • Educate patients and caregivers and the health care team in the phases of occupational therapy in which they may assist the patient, in the use of adaptive devices, and durable medical equipment, as appropriate.
  • Provides staff in-services education as directed by the Clinical Manager.
  • Assess and regularly re-evaluate the needs of the patient, develop, implement and revise the patient’s plan of care.
  • Recommend special equipment and instruction on proper use to patients and families.
  • Initiate diagnostic, preventive and rehabilitative procedures as appropriate to the patient’s care and safety.
  • Assist with development of the plan of care and maintain continuity of multidisciplinary patients whenever needed, to ensure coordination of care.
  • Promote coordination of care through interdisciplinary case conferences and communication with the primary care provider and clinical staff.
  • Develop, prepare and maintain individualized client care progress records with accuracy, timelines and according to policies. Submit accurate documentation in accordance with agency policies.
  • Keep abreast of occupational trends and knowledge for service provision, documentation and care coordination. 
  • Participate in staff meetings, in-service programs and Utilization reviews as requested.
  • Participate in the agency’s quality improvement program.
  • If applicable, supervise the Occupational Therapy Aide at least every 30 days.
  • Travel to a variety of patient homes and perform in conditions that vary greatly depending upon the patient’s home environment. Some homes will be clean, neat, and maintained at a comfortable temperature. Other homes may be cluttered, dirty, with an uncomfortable temperature. 

REQUIREMENTS:

EDUCATION:                                     

  • Master Level Occupational Therapy degree from an accredited educational program

CERTIFICATION/LICENSURE:      

  • MN registration; license in good standing

EXPERIENCE:                                     

  • At least 1 year of experience in Home Health, acute care, transitional care or outpatient clinic setting

KNOWLEDGE, SKILLS, ABILITIES:

  • Ability to demonstrate and practice the principles of proper body mechanics for all physical activities
  • Ability to practice universal precautions, standards and facility infection control procedures
  • Knowledge of and ability to use position related
