Primary Function/General Purpose of Position
Coordinates Every Woman’s Life program and high-risk breast services for Bon Secours Hampton Roads. Responsible for the implementation and fulfillment of the EWL contract which offers breast and cervical cancer screening and diagnostic services, client education, case management, and tracking and follow-up to enrolled individuals. Additionally, the role acts as a liaison between women’s radiological services and the breast surgery department to identify individuals at a significantly increased risk of developing breast cancer due to family history, genetic mutations, or other factors, to provide patients with resources/services such as enhanced screening methods, personalized risk assessments, and potential preventative treatment options including genetic counseling and close monitoring by a dedicated team of healthcare professionals
Essential Job Functions
Every Woman’s Life Case Management and Coordination
A. Ensures that Bon Secours Hampton Roads’ Program meets screening and fund expenditure targets as established and effectively communicates program information from State EWL program to all staff participating in the program.
B. Requests reimbursement from the State EWL office, monthly, and within 90 days of the initial screening date using appropriate forms.
C. Ensures all reporting and data requirements are met.
D. Coordinates care for all EWL enrollees with an abnormal screening or diagnostic result until a final diagnosis is made, and if needed, treatment is initiated by:
· Completing a client needs assessment within five business days of receiving an abnormal result to identify any client barriers, such as transportation, scheduling, and lack of understanding of the nature or purpose of a follow-up procedure.
· Developing a comprehensive care plan and providing appropriate referrals for clients with identified needs to remove service barriers so the client can act on recommendations and receive the follow-up procedures they need.
E. Implements and maintains client tracking and reminder system.
F. Provides age-appropriate referrals for other medical or cancer screening services as well as referrals to promote and encourage a healthy lifestyle.
G. Monitors and tracks the client’s clinical status throughout the course of enrollment, even if services are being provided by a sub-contractor.
H. Ensures documentation submitted to the state office for invoice aligns with the documentation in the client’s medical record.
I. Represents Bon Secours EWL program at appropriate healthcare screening and education events.
J. Attends annual and ad hoc EWL training.
High-Risk Breast Coordination
K. Acts as an advocate for high-risk breast cancer patients identifying, notifying, and navigating patient through the various resources, testing and monitoring.
L. Educates patients on how family history, genetics, imaging, and labs help determine breast cancer risks and resources and services available for monitoring and early detection.
M. Coordinates patient paperwork needs, monitors, and manages Epic in-basket requests, triages incoming critical results, and monitors for new symptom management needs.
N. Implements and maintains thorough client tracking and reminder system.
O. Communicates and coordinates care among broader multi-disciplinary team.
P. Participates in review of quality metrics to maximize outcomes, collects data, and prepares various reports pertinent to the department, especially when pertaining to accreditations regionally and site specific. Assists with quality improvement activities.
Licensing/Certification
Current Registered Nurse license in the work state (required)
Current CPR/BLS (required)
Education
High school graduate (required)
Post high school education (required)
Bachelor’s Degree (preferred)
Work Experience
Previous experience as a manager or program coordinator required. Oncology experience preferred.
