Responsibilities:

- Implement and maintain established nursing practice objectives and standards.
- Review hospital, skilled discharges, physician orders, and care need to ensure safe implementation in community setting.
- Evaluate resident health status and identify significant changes in conditions requiring registered nurse interventions and provider notification.
- Implement the registered nurse delegation process for unlicensed assistive personnel.
- Evaluate the resident’s condition and needs to determine which tasks can be safely delegated.
- Assess competence and training of the delegate to ensure the unlicensed staff can perform the delegated task safely.
- Assist with development and review of resident service plans related to health conditions and nursing needs.
- Delegate specific nursing tasks to appropriate staff in accordance with state regulations and community policies.
- Provide education, written instructions, and demonstration of delegated nursing tasks to staff. Ensure clear and concise instructions to the delegate, including specific procedures, expected outcomes, and potential complications.
- Observe and validate staff competency prior to authorizing delegated tasks.
- Monitor the delegate’s performance of the delegated task, providing guidance and support as needed.
- Evaluate the outcome of the delegated task to ensure that it meets the resident’s needs and that the task was performed safely and effectively.
- Document all delegation-related information, including the tasks delegated, the delegate’s competence, and the outcomes of the delegated tasks.
- Collaborate with other health care professions, such as physicians, nurse practitioners, physician’s assistants, and therapists, to ensure a coordinated approach to patient care.
- Provide ongoing education and training to the clinical team about delegation principles and procedures, medication administration, infection control, chronic disease management, and emergency response procedures.
Requirements and Qualifications
Education
- Registered Nurse license or greater, in good standing with the Board of Nursing in the practicing state
- Experience in an Assisted Living and Memory Care setting is preferred
Experience
- Knowledge and experience caring for and interacting with elders
- Must have a passion for and a desire to help others
- Maintain confidentiality of all resident care information in accordance with HIPAA guidelines
- Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy
