Position Overview

The Case Manager will determine if the member/consumer is appropriate for Case Management (CM) Services. Case Manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the member’s health and human service needs.

Job Responsibilities

  • Holistic Care Coordination: You'll take charge of coordinating care between all medical and behavioral health service providers. This includes the preparation of Transition Summaries for Fee For Service members who are transitioning to enrollment with a Managed Care Organization (MCO).
  • Individualized Care Planning: You'll conduct patient assessments and evaluating data and statistics obtained from ongoing review processes. Assess the unique needs of our patients/members and develope indviualized care plans that are tailored to their specific needs and requirements. Communication and design of services that align with West Virginia Fee For Service Members and Providers.
  • Team Collaboration: Working closely with the Case Manager Lead and other members of the Case Management team, you'll determine when member goals have been met and are ready to be discharged from the Case Management Program.
  • Utilization of Cutting-Edge Tools: You'll employ advanced tools such as Social Determinants of Health (SDOH) and leverage additional Case Management resources available through the Atrezzo system to provide comprehensive care to our members.

Required Qualifications, Experience and Knoweldge

  • Minimim of 4 + years of postgraduate experience from an accredited school of nursing, and you must currently hold a Registered Nurse license in the state of West Virginia.
  • 2 - 3 years of prior experience in a Case Management position. This background equips you with the skills and insights needed to excel in this role.
  • We're looking for individuals who possess strong analytical abilities. You'll be responsible for identifying and assessing problems and evaluating data and statistics obtained from ongoing review processes.
  • Certification in Case Management and BSN highly preferred. These credentials demonstrate your commitment to excellence in case management.
  • Effective and compassionate communication skills are a must. Your ability to connect with patients and team members is essential for delivering quality care.
  • Successful Case Manager will have excellent organizational and time management skills, allowing them to manage multiple priorities effectively.
  • Staying up-to-date with emerging professional and technical knowledge is crucial in our field. Familiarity with the latest advancements is highly valued.
  • We're seek individuals who excel in problem-solving, possess the ability to multi-task, and are driven to deliver solutions. Compassion and teamwork skills are essential. Your ability to collaborate effectively with colleagues and patients is vital for our healthcare mission.
  • Proficiency in navigating computer and software applications, given the reliance on technology in healthcare.
  • Location Requirement: While remote work flexibility is available, it's important to note that candidates must be based and reside within 45 miles of the West Virginia border to ensure timely and effective support for our patients in this region.
