PRN Physical Therapist

Our partner a modern physical therapy practice is seeking a skilled and compassionate Physical Therapist to join their clinical team in New York, NY. The practice aims to restore mobility and confidence for individuals dealing with chronic pain, surgery, or injury. By leveraging a combination of in-person and telehealth care, along with innovative digital tools, this practice aims to improve patient matching, adherence, and satisfaction.

As a Physical Therapist, you will play a vital role in delivering exceptional clinical care through virtual telehealth appointments and in-person visits at patients' homes. This is an excellent opportunity to contribute to the transformation of patients' lives by expanding access to effective treatments for musculoskeletal disorders.

Responsibilities:

• Provide clinical care to patients through virtual telehealth appointments and in-person visits at patients' homes.
• Develop individualized treatment plans, exercise prescriptions, and deliver necessary care to assigned patient caseload.
• Ensure a high level of clinical care and deliver an exceptional patient experience.
• Document all treatments accurately and efficiently in electronic medical records (EMR).
• Communicate effectively with patients via chat as needed.
• Maintain a minimum availability of 8 hours per month in either an in-home or telehealth setting.

Requirements

• Licensed Physical Therapist with a minimum of 3 years of clinical experience (preferred).
• Completion of state-regulated Continuing Education Units (CEUs).
• CPR certification.
• Excellent written and verbal communication skills with a compassionate demeanor in virtual interactions.
• Passionate about contributing to a new model that enhances clinical outcomes.
• Demonstrates creativity, flexibility, and a patient-centric approach.
• Proven ability to build successful and respectful relationships with the entire team.
• Bonus: Previous experience in home health or telehealth.
