GENERAL DESCRIPTION:
The Project BUILD Outreach Worker will work with youth and young adults, ages 10-21, and their families. The position will carry a regular caseload of 15-25 gang-involved program participants on an ongoing basis. This position will respond to referrals, conduct intake assessments, and work as a part of a multidisciplinary team to develop an intervention plan and implement this plan with clients. This position requires frequent interaction and relationship building with youth, families, and community partners, including schools, court and criminal justice personnel, and grassroots agencies. The Project BUILD Outreach Worker will have a high level of familiarity with Durham neighborhoods, agencies, and schools; knowledge of local gang issues; and prior experience working with youth. Durham County residents preferred.
DUTIES AND RESPONSIBILITIES:
The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.
- Work with youth and young adults, ages 10-21, and their families;
- Carry a regular caseload of 15-25 gang-involved program participants on an ongoing basis;
- Provides intense case management which includes home visits and transporting participants;
- Responsible for linking these individuals to services and support;
- Work closely with gang involved youth to assist them with achieving educational success, addressing social skills deficits, coordinated service delivery, advocacy and gaining employment;
- Assist youth with enrolling in school, maintaining appropriate school behavior and attendance, and connecting them with other needed services;
