Job Summary:

Assists with operations of the Adult Foster Care Program. In collaboration with supervisor, ensures AFC program meets regulatory requirements. Is responsible for training, delegation, and ensuring staffing needs meet the requirements of the day-to-day operations. Contributes to the accomplishment of the Tribe’s holistic health and wellness objectives for patients and families.

Essential Functions:

  • Assists in the management of the Adult Foster Care (AFC) resident services; adhering to regulatory principles and the oversight of policies and procedures for the care of the elder and disabled residents.
  • Coordinates AFC activities in accordance with policy and procedure.
  • Maintains and provides all data as requested and/or required.
  • Accurately records resident information in designated documentation systems.
  • Responsible for nursing, and caregiving staff competency-related activities to ensure safety and quality within the AFC.
  • Collaborates with the supervisor for job descriptions, hiring and retention of AFC nurses and caregiving staff.
  • Provides clinical education and skill delegation to AFC staff.
  • . Coordinates patient care with other departments/programs.
  • Conducts assessments of potential residents and develops service plans, schedules and updates service information as indicated by resident need.
  • Engages resident and family participation in service/care planning by explaining approaches and involving them in teaching programs. Conducts (or ensure occurrence of) on-going care conferences/assessments as per policies and procedures with family members/resident representatives.
  • Monitors residents’ physical and mental functioning psychological well-being and nutritional status to ensure care needs are met in accordance with resident service plans. Notifies manager, physician and family and takes appropriate action when significant changes in condition occur.
  • Participates in written plan of care for each resident and reviews resident’s medical nursing treatments to ensure that are in accordance with the resident’s care plan and wishes.
  • Performs or coordinates direct resident care as necessary or appropriate.
  • Assists with monthly resident meetings regarding the operation and governance of the AFC facilities.
  • Collaborates and coordinates the monthly AFC staffing schedule with the AFC Administrative Assistant to ensure adherence to policy and procedures and resident care needs are met at all times.
  • Provides staff oversight to ensure safe practices are maintained per policy and procedure and reports back to supervisor/manager when concerns arise.
  • Assists with new caregiver onboarding and ensures all tasks are signed off prior to the completion of training onboarding; as well as completes annual skills checklists with caregivers.
  • Performs a variety of other duties as assigned by supervisor. 

Education, Experience, and Licenses:

  • Requires a current BSN or ASN from an accredited college or university.
  • A minimum of three (3) years’ experience as a Registered Nurse with a minimum of one (1) year experience in a clinical setting preferred.
  • Current Oregon Nursing License is required.
  • Current CPR/BLS is required.
  • OR equivalent combination of education, training and experience. 

Driving Requirements:

  • This position may require driving. 

Qualifications, Registrations, and Certifications:

  • Demonstrated proficiency in wound care, phlebotomy, and catheter placement required.
  • Must be detail oriented with ability to handle high patient volumes.
  • Must have excellent customer service skills.
  • Must have knowledge of Registered Nursing scope of practice.
  • Ability to operate and maintain medical equipment within the Adult Foster Care.
  • Must have knowledge and experience in use of computerized electronic health records and software applications.
  • Working knowledge of sterile technique and special procedures applicable to care delivered.
  • Requires working knowledge of sanitation, personal hygiene, and basic health and safety precautions applicable to health clinic setting.
  • Requires ability to explain and discuss difficult information, establish rapport, exercise patience and gain understanding.
  • Adherences to the organization’s policies and procedures covering the privacy, access, and patient health information in compliance with Federal and state laws.
