FlexStaff is seeking an Occupational Therapist (OT) for our client, a nonprofit geriatric health and rehabilitation system that provides skilled nursing, short-term rehabilitation, and home health programs for older adults of all backgrounds. This is a full-time, Monday-Friday.
Qualifications:

Must possess a current, unencumbered license to practice as Occupational Therapist in the State of New York.

Successful completion of an occupational therapy program at a school of occupational therapy approved by the American Occupational Therapy Association or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association.

Master’s degree or Doctorate degree in Occupational Therapy.

One year of occupational therapy experience preferred.
You will be responsible for completing comprehensive patient evaluation, developing and implement individualized treatment plan for a variety of patient presentations, modifying treatment plans and goals based on patient response, establishing measurable goals with achievement dates.
Duties and Responsibilities:

Collaborates with physicians, patients, families, and the care team to develop and implement personalized care plans.

Utilizes various methods to educate and promote patient health and independence.

Assesses, monitors, documents, and reports on patients' health progress as required.

Supervises aides and physical therapy assistants, observing and reporting any changes in patients’ conditions.

Completes OASIS assessments and documentation accurately and promptly, forwarding as needed.

Supervises occupational therapy assistants per state regulations and agency policy.

Coordinates the total care plan and maintains continuity by liaising with other staff involved with the same patients, participating in case conferences as necessary.

Performs additional duties as assigned.
Eligible newly hired OT’s will recieve a $6,000 hiring bonus, paid in two phases: $3,000 after six months of employment and another $3,000 after one year of service.
