Authorization Case Manager - Registered Nurse (RN)

About the role

This is a remote position - candidates must reside within 2 hours of an AOC branch office. The RN Authorization Case Manager provides professional nursing services to address the social and emotional factors impacting a client’s ability to achieve the goals outlined in their Plan of Care. Working under the direction of a physician and in collaboration with the interdisciplinary health care team, this role ensures seamless coordination of pediatric patient services, engaging patients, families, and referring agencies to deliver quality, compassionate care in accordance with established facility policies and procedures. 

Responsibilities:

Patient Assessment & Care Planning
  • Educate and evaluate clients under an approved Plan of Care.
  • Complete timely assessments per facility policy.
  • Develop and maintain care plans for the health team.
  • Document findings, interventions, and progress.
  • Communicate updates to physicians, supervisors, and staff.
Social & Emotional Support
  • Prepare social histories to guide care, considering family, cultural, emotional, and financial factors.
Collaboration & Communication
  • Work effectively with the team to meet patient and facility goals.
  • Participate in meetings and case conferences.
  • Educate on community resources and proper access.

Qualifications

  • Current Registered Nurse (RN) license in good standing with the State of Texas.
  • Graduate of an accredited nursing program.
  • Valid driver’s license and current auto insurance.
  • Preferred: Home Health, pediatric, and/or office nursing experience.
