The Clinical Lead provides guidance and support to the clinical team in our primary care clinic, helping to ensure smooth daily

operations and high-quality patient care. This role involves offering expertise in patient care, mentoring

staff, and facilitating communication between the team and management.

  


Requirements

QUALIFICATIONS

Education/ Experience: 

  • Graduate of accredited school of nursing
  • Current LPN or Registered Nurse licensure in Vermont
  • 3-5 years’ experience in primary care and/or emergency department, preferred
  • Current Basic Life Support certification
  • Excellent critical thinking and judgement skills
  • Excellent customer service skills
  • Experience with Electronic Medical Records Systems

Essential Functions Of The Position:

  • Maintains daily operations of the clinic team. Including but not limited to, daily huddles, oversight of clinical staff, provider and patient flow. 
  • Ensures pre-visit planning is completed, and patient charts are populated correctly. 
  • Manage Point-of-Care testing and supplies
  • Ensures Clinic and Population Health Manager is informed of any issues with patients, staff, or physicians
  • Responsible for the oversight and appropriate supply of all vaccines (GCFH and VDH)
  • Collaborates directly with Clinic and Population Health Manager and Senior Director as needed.
  • Collaborates directly with clinic providers for day-to-day patient needs.
  • As part of population health management is responsible for facilitating quality care in accordance with established clinical practice guidelines, protocols and provider direction. 
  • Fosters patient engagement and adherence to plan of care to achieve evidence based clinical objectives and goals specific to chronic conditions and preventive health measures.
  • Attends PCMH or optimization meetings as needed.
  • Communicates with providers, clinical team and the Clinic and Population Health Manager regarding any urgent needs.
  • Oversight and management of clinical pools.
  • Assist and provide guidance or recommendations for patient triage.
  • Enter or update all patient information into EMR in accordance with established protocols and procedures.
  • Document all patient interactions accurately, timely and appropriately.
  • Assists with routine medication refills, as needed. 
  • Obtains and processes various laboratory specimens according to established protocols and procedures. Performs point-of-care testing, as needed.
  • Perform basic medical procedures such as dressing changes, spirometry, ear irrigation, EKG, audiometry and vision screenings, as needed
  • Participates in quality improvement, management, continuing education, and other patient care programs established by Grace Cottage Family Health & Hospital.
  • Assists in the resolution of complaints, requests and inquiries from patients.
