The Village at Summerville is seeking a highly skilled and compassionate Director of Social Services to join our team to create a warm hospitality experience for all our residents, staff and visitors. In this position, you will enjoy working with a passionate team to provide quality service to residents in our Independent Living, Assisted Living and Memory Care settings. 


What You'll Do:

  • Uphold the PCSC Mission, Values, and Commitment Statements in all aspects of work, leading by example and holding self-accountable for exemplifying compassionate Christian service. 
  • Assist residents with their adjustment to their new living conditions and community. 
  • Maintain compliance with all regulatory agency requirements for Social Services. 
  • Implement and conduct resident orientation for IL/AL/MC.
  • Educate residents on the importance of having POA/DNR.
  • Encourage family contact and communicate with families on an as needed basis; assist residents with various forms of communication to family. 
  • Assist residents and their families with future planning and provide education on aging process.
  • Review Bill of Rights with Residents and their families; obtain appropriate signatures. 
  • Review, schedule and attend AL/MC Care Plan meetings with appropriate parties and obtain appropriate signatures.
  • Gather social information related to resident. 
  • Discuss concerns, as appropriate, with Chaplain and/or Executive Director and collaborate on support groups as necessary.
  • Document in Point Click Care.
  • Maintain Awareness of current community resources available to residents, families, and staff members (Vital Living, Home Health, Hospice, Therapy, etc)
  • Coordinate resident transitions from one level of care to another.
  • Review grievances and lead Resident Risk and provide proper follow up.
  • Maintain Trust funds accounts as assigned. 
  • Monitor personal appearance of residents, recommending needed changes in wardrobe, hair and general appearance to the appropriate staff member. 
  • Oversee podiatry, massage therapy, beautician, audiologist, other outside vendors for IL/AL/MC.
  • Annually update Medicare cards, emergency contact information, etc.
  • Follow up on all Independent Living incidents.
  • Attend meetings as assigned.
  • Perform other duties as assigned. 


What We Are Looking For:

  • Bachelor’s Degree in Social Work 
  • 1 year of social work experience in long term care setting 
  • Intermediate computer skills 
  • Valid SC Drivers License  

Preferred Education, Experience, and Skills:

  • Master's Degree in Social Work
  • Licensed as a Social Worker in South Carolina
  • 3 years social work experience in a long term care setting
