Care Manager, Remote Patient Monitoring (RPM) will be responsible for the hands-on execution of clinical and sub-clinical services and operations for Clover Health Partner’s aligned beneficiaries enrolled in remote patient monitoring programs (ex: CHF, Hypertension, Diabetes, COPD). Leveraging data from the RPM, the Care Manager will outreach to patients via phone when they report an escalation in their symptoms and provide proactive care for patients according to their long-term risk status. The Care Manager will follow standard operating procedures (SOPs) which may include care coordination, communication coordination/escalation to providers, alert management, and risk management for patients.
As a Care Manager, Remote Patient Monitoring (RPM), you will:
- Use Clover’s RPM tool and data to provide virtual care for large volumes of patients.
- Follow Standard Operating Procedures (SOPs) as defined by Clover Health Partners’ provider groups to provide care to patients.
- Use clinical judgment to escalate patients to providers as needed and as defined in SOPs.
- Build trust, utilizing motivational communication techniques and strategies, to help drive positive health behavior changes as defined by the provider engagement SOP.
- Serve patients by proactively monitoring, educating, and coaching patients on a plan for better health.
- Assists patients with remote patient monitoring adherence and program participation.
- Outreaches to patients and/or providers when vital signs are out of range or not transmitted telephonically or via video conference and follows an escalation pathway.
- Educate patients on how to manage their acute or chronic medical conditions, with a goal to maintain health, wellness, and to avoid emergency room utilization and hospitalization (as defined by the provider engagement SOP).
- Document visit findings in software platform.
- May engage in clinical practice transformation efforts to support peaks in demand (manage practice Annual Wellness Visit engagement, cost and quality improvement initiatives, chronic condition assessment, documentation and care planning).

You should get in touch if:
- 3-5 years post-degree experience in a direct health care setting (including either physical or virtual care).
- An active, unencumbered, RN license in one or more of the following the states of FL, GA, IL, MO, OK, NJ or NM is required.
- Considerable skill in interpreting clinical treatment information and making appropriate referral and triage decisions.
- Ability to use various software programs on different standard operating systems; ability to learn quickly.
- Ability to communicate clearly and professionally in a range of circumstances.
- Intermediate level of proficiency with Mac/PC computer skills.
- Ability to handle stressful situations.
- Ability to work independently.
- Preferred but not required: Spanish speaker.
