Job Description:
As the Lead Transitional Housing Counselor, you will ensure the safe and orderly provision of Transitional Housing services to all clients in residence coordinating their case management with the TH Case Manager. Develops and/or reviews individualized treatment service plan for assigned clients. Monitors client’s progress, documents progress toward their goals. Follows the progress of assigned clients to ensure they are following through on their recovery program. Coordinates the delivery and monitors all services provided internally or externally for each client. Responsible for maintaining a caseload and conducting individual, family and group counseling as noted in the treatment plan. Teach educational classes as needed. As the Lead Transitional Housing Counselor, you are responsible for ensuring that the Mission of The Salvation Army is effectively carried out.
What You Will Do:
- Assists in maintaining a structured environment for clients and staff
- Prepares and/or reviews the treatment plan in accordance with the Biopsychosocial assessment, ASAM criteria and Initial Treatment Plan. Reviews goals on an ongoing basis and makes changes as needed for the client
- Provides counseling to individual clients in accordance with goals developed on the treatment plan. (Individual counseling can be conducted by using cognitive therapy, psychoanalytic therapy, reality therapy Gestalt therapy and behavioral therapy)
- Facilitates group counseling sessions and follows the assigned group format for each session
- Prepares and teaches educational classes as assigned
- Documents all client contacts in the electronic medical records system, including progress notes, treatment plans, monthly reports, discharge summaries and discharge plans
- Conducts regular staff meetings and trainings for staff. Participates in departmental and agency staff meeting as scheduled
- Ensures the accurate reporting and collection of client housing fees and collaborates with the Finance Department regarding collections and evictions
- Maintains and provides statistical and other reporting as required
- Coordinates, manages, and monitors client phase requirements and assists client to develop individualized phase outline plan
- Conducts follow-up contacts
- Provides support, encouragement, correction, and compassion to client as needed
- Assures client transportation as needed (i.e., bus tickets)
- Provides case work and intervention for non-addiction issues, as needed and appropriate
- Conducts outside assessments for clients as assigned and Withdrawal Management within 48 hours of admission, as applicable
- Maintains client confidentiality of all client information according to t Federal, State and The Salvation Army standards, policies, and procedures
- Ensures adequate and efficient coverage for facility operations in scheduling
- Chairs the Transitional Housing Committee. Required to serve on the Safety Committee and Quality Assurance Committee. Attendance is required for Safety Meetings and other responsibilities communicated by Quality Assurance Manager
- Facilitate and lead client visitation services per The Department of Child Services guidelines. Monitor The Department of Child Services (DCS) correspondence ensuring that all documentation is provided to the provider in a timely manner
- Conducts urine screens for clients according to policy and procedure
- Other tasks as assigned by leadership
Minimum Qualifications: The requirements listed below are representative of the minimal education, experience, skills, and/or abilities required for this position.
Education: Bachelor’s degree in social or human service areas required. Master’s degree in human services or other related field preferred
