About the role
The Director, Pharmacy Outreach will be responsible for the overall strategic direction and operation of the centralized pharmacy outreach program. This role will focus on activities that contribute to success in value- based care contracting: improved quality performance, better patient outcomes, and lower total costs of care (including medication adherence support, pharmacotherapy optimization, and disease state management).

Responsibilities:
- Pharmacy Operations: Collaborates with key stakeholders to provide leadership for operational effectiveness. Works with Operations Manager and Product Team to optimize workflow processes. Responsible for monitoring and achieving key performance metrics. Responsible for pharmacy program budget and staffing model.
- Clinical Service Design: Collaborates with Physicians, practice staff, Aledade Provider Networks team, and Aledade Product teams to optimize medication management workflows and communication, patient experience, and practice satisfaction leveraging both clinical and operational expertise.
- People Management: Builds and develops a highly skilled pharmacy team, particularly focused on supporting a small number of skilled clinical pharmacists who can collaborate with pharmacy technicians and other call agents. Collaborates with the Quality Learning and Development team to build rigorous quality assurance processes.
- Innovation: Identifies novel opportunities to contribute to Aledade quality and savings priorities; contributes to design and execution of innovation tests
Requirements:
- Doctorate of Pharmacy degree; PGY1 pharmacy residency preferred
- Current and valud licensure in state of residence
- 12 yrs in discipline; 6 -8 yrs. of management experience in a clinical pharmacy setting
- Experience with value based contracting and Medicare Advantage STARS
- Strong collaboration skills
- In depth knowledge of pharmacotherapy and pharmacy practice
- Demonstrated leadership ability: Ability to motivate and coach employees, promote ongoing learning and lead improvement activities.
- Excellent communication skills, both verbal and written
- Ability to work effectively in a matrixed, cross-functional environment
