Physical Therapist (PT) – Skilled Nursing & Rehabilitation | Kingston Care Center of Sylvania, OH

PRN | On-Site | Ohio PT License Required

Kingston Care Center of Sylvania is seeking a licensed Physical Therapist (PT) to join our skilled nursing and rehabilitation team. In this role, you’ll help residents recover strength, mobility, and independence while working in a supportive, team-focused environment. Whether it’s guiding someone through post-surgery rehab or helping a long-term care resident maintain quality of life, your expertise will make a lasting difference right here in Sylvania, Ohio.

What You’ll Do as a Physical Therapist

• Evaluate resident abilities and design personalized therapy plans

• Provide one-on-one therapy to improve mobility, balance, strength, and daily function

• Monitor and document progress, updating plans as needed to achieve resident goals

• Collaborate with physicians, nurses, and therapy colleagues for coordinated care

• Educate residents and families on therapy goals, safe movement, and continued exercises

• Ensure therapy delivery meets state regulations and facility standards

What You’ll Need

• Active Ohio Physical Therapist (PT) license required

• Graduate degree in Physical Therapy from an accredited program

• Experience in skilled nursing, rehabilitation, or long-term care preferred (new grads encouraged to apply)

• Strong clinical judgment, communication, and organizational skills

• Compassionate, resident-first approach to therapy

• Ability to work independently and as part of a team
