Job Description Summary

The Lead Psychiatric Social Worker (MSW) reports to the Social Work Manger of the Institute of Psychiatry – Mental Health ICCE. This position is approximately 80% clinical direct patient care and 20% administrative.

Under clinical practice guidance of the Social Work Manager, the Psychiatric Social Worker Lead assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes a psychosocial assessment on the patient and caregivers throughout the hospitalization and arranges outpatient follow-up as needed. This position provides a wide variety of direct clinical social worker services in an acute care psychiatric setting for a complex caseload of patients including psychosocial assessment, active treatment (group therapy), discharge planning, and case management. The Social Worker identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.

Under general supervision and in coordination with the Social Work Manager, the Lead Psychiatric Social Worker provides professional supervision and direction to the Social Worker team to achieve the best clinical outcomes for patient groups and/or clinics. This position serves as a clinical expert and resource related to social work, clinical care, discharge planning, and care transitions.

Requirements: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Experience: Five years post graduate professional work experience in social work practice. Credentials: LISW-CP licensure by the South Carolina Board of Social Work Examiners required. Requires excellent communication skills, attention to detail, and ability to work independently.  Demonstrates ability to collaborate in a positive, professional manner with medical professionals.  Requires ability to work in a thorough but efficient manner. Certified in Safety Care and Basic Life Support within the first 90 days of employment and annually thereafter.
