This position is responsible for providing individual clinical and mental health consultation to long term home visitors on a regular basis, as needed, or in a crisis situation. This position also provides regular training to all staff members on topics related to difficult issues that families experience such as mental health, substance abuse, and family violence.
Consultation is done in the previous of the assigned supervisor in order to provide modeling and training in reflective practice and supervision. The consultant enters all required information in data management system and reviews/approves report before monthly invoice is submitted to Department of Child Services for reimbursement and in accordance with the grant contract.
Clinical Consultation
Meets with home visitors and their assigned supervisor monthly for one hour and within 30-45 days for all newly assigned families. Those home visitors with blended caseload where only one or two families are on his/her caseload, the duration of the consultation may vary based upon the needs of the family or the home visitor. The priority of this consultation will be determined by the supervisor and the home visitor with priority given to the highest risk families.
The documentation includes the clinician's assessment of the family's needs, the suggested/planned interview strategy, and the resulting following up on the success of the planned intervention.
Attends weekly case assignment meeting where the assessment and depression screening is shared with the members of the management team in order to identify families who would benefit from mental health consultation and determine the family is assigned to the home visitor with the appropriate skill set and cultural characteristics. Documents family numbers of high risk for child abuse and neglect and elevated depression scoring in order to research the family's record during office time and may change risk to clinical high risk based on clinical expertise.
Supportive Home Visits
Accompanies home visitors on family home visits to provide immediately clinical intervention and assists with referral coordination for on-going clinical intervention services beyond home visiting.
Ongoing clinical services are not part of the home visiting model and care should be exercised to not imply that clinician or home visitors will be providing clinical services.
Apply a minimum of one health literacy method when communicating with patients/clients.
Training
Conduct training (minimum bi-monthly) to address overall trends and concerns related mental health, and mental health training as requested by home visitors, supervisors or program management.
Monitor and identify trends related to mental health concerns and reports concerns to program management, participates in evaluation of mental health consultation as requested by program management or DCS.
Documentation and Completion of Required Forms
Prepares clinical notes reflecting the consultation and gives a copy of the documentation to the supervisor for filing in the home visitor's supervision binder.
Documentation includes the development and review of intervention plans, documentation of the family response to the intervention and the resulting success of the action plan. If the plan was not successful, the documentation will reflect the problem-solving and intervention planning strategies that were discussed and documentation in the future meeting.
